Medical Simulator: The inception of uprooting the curse of medical error

A medical error is a preventable adverse effect of care, whether or not it is evident or harmful to the patient. This might include an inaccurate or incomplete diagnosis or treatment of a disease, injury, syndrome, behavior, infection, or other ailment. For medical errors, the generally quoted Heinrich's ratio is 600:30:10:1, i.e. for every 600 non reportable incidents there are 30 reportable incidents, 10 accidents, and one fatal accident. According to the Institute of Medicine, 44,000 to 96,000 deaths annually are recorded due primarily to medical mistakes during treatment. The reason behind medical errors are: lack of prior exposure to rare events like emergencies and unusual clinical syndromes, lack of application of theoretical knowledge to an individual patient, critical events not supervised by senior staff, inadequate debriefing, inadequate exposure to rare and complex major clinical situations etc.
To overcome these problems, the doctors need more practices so that they can be ready for any kind of medical emergency and situation. Simulator can play a great role here. There are different types of simulators, for example- cardiac disease surgery, endoscopy, catheter simulator, child birth simulator, haptic simulator, anesthesiology, bronchoscopy etc. But the main problem behind using these simulators is, they are too much expensive for which the medical centers can hardly afford to buy it in the low and middle income countries like Bangladesh. So research should be reinforced to develop simulators cheaply.
We have already developed a heart and lung sound simulator in our undergraduate project which is way cheaper than the existing simulators in market. More should be introduced so that the trainees and medical students could get the best training and get expertise. Conducting the simulation exercise after the roleplay is a more effective teaching method for a deeper understanding of clinical situations and improved self-efficacy.
